Virtual Consultation

Due to the changing world of healthcare and technology, we now have the ability to provide our patients with Virtual Consultation.

We strongly believe in protecting the privacy of our patients when they are sending important photos and videos necessary to assist the dentist to conduct a successful a virtual consultation that will address the patients concern. When you provide this information to us, it is only used for the purpose of consultation. In order to protect your privacy all photos and videos are to be sent thru a secure-mail. We do not share the names, e‐mail addresses, and/or telephone numbers of patients with any other companies, or with any other patient. 

By placing my signature below, I acknowledge that I have read and understand the above statement on sending photos and videos for Virtual Consultation. I hereby give permission to Mission Dental Arts to call or video conference me to conduct a Virtual Consultation. I am aware that my dental insurance may or may not cover the $69 fee for the virtual consultation and will be responsible if it is not covered.
